Pharmacists may make mistakes if they confuse one medication for another. They may also provide you with the wrong dosage of the correct medication. In some cases, pharmacists are simply not paying attention to what they are doing. They may be overworked if they are employed at a very busy pharmacy.
The error may also occur if pharmacists receive incorrect information from a physician. Doctors sometimes order the wrong medication. They might also write an illegible prescription. This could cause the pharmacist to provide you with the wrong medication.
